New Patient Registration
To give the best possible care to our patients we have to restrict the area within which we can register people. We therefore have a defined practice area from which we accept new patients and a map of our practice area can be found in reception.
We have a larger outer area where existing patients can move to and still remain on our list. If patients move outside this area we will ask them to register elsewhere. This is sometimes upsetting, especially to patients who have been with us a long time but we do have to set limits to our practice area.
To register as a patient please bring proof of identity (passport/birth certificate) and address in the form of a utility bill or bank statement not more than 3 months old with you when you come to register.
All new patients registering will be asked to complete a patient questionnaire to help us offer the best service.
Please bring your child’s Red Book when you come to register them.
