You will see me if you have a long term condition for your annual review, specifically in Asthma (adults and children over 12), COPD and Hypertension.
If your medication or dose needs changing, I will guide you through the process.
I can answer your medication queries, for example if you are experiencing side effects, through telephone and face to face appointments that can be arranged by calling the surgery. 
I will review all your medications if you have been in hospital to ensure they are up to date.
I will also be in touch if your regular blood test is due, arrange testing and explain if or how your repeat prescriptions are affected.

A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
